After a data breach, how fast must a business tell people, and who else must it tell?

The general breach notification law for businesses in all 50 states and DC, from official state pages where we can read them. Laws for banks, insurers, health care and government often differ and are left out. A blank means the page we could read doesn't say, never that the answer is no, and four states are blank throughout.
